The purpose of the survey is to gather public opinions about health issues in Delaware County. The information will be used to identify the most important areas for health improvements that can be addressed through community actions. Participation in the survey is voluntary and all responses are completely anonymous. In this process, your voice and input are vital. We thank you for your participation.
The information gathered from your survey can help develop a community health improvement plan by justifying how and where resources should be allocated to best meet community needs.
